Who can fill in the form for your staff, if your company is performing an essential or permitted service?
Any person performing an essential or permitted service must be designated in writing by the head of an institution (or a person designated by him or her) on a form that corresponds to this Permit to Perform an Essential or Permitted Service. Click here to download the form.
“Head of institution” is defined by the Regulations as the accounting officer of a public institution and the chief executive officer or the equivalent of a chief executive officer of a private institution.
“Institution” means any public or private institution, including a sole practitioner and any other business owned and operated by a single person, that is engaged in the supply or distribution of a good or service as set out in Table 1,* or which regulates such supply or distribution, including professional regulatory bodies designated in directions made in terms of Regulation 4.*

As an extra precaution, it is recommended that institutions use an official stamp and/ or unique features that would make forgery more difficult and that photocopies be avoided. The carrier of the Permit must ensure to keep identification ready at all times.
